Trial Class Agreement

I acknowledge that participation, training, attendance and/or competition in martial arts involves, at times, intense bodily contact and I assume any and all risks while I am present at the Brazilian Martial Arts Center as a participant, competitor and/or a spectator. I agree to indemnify and hold harmless Marcelo Siqueira, the Brazilian Martial Arts Center, its agents, servants, and/or employee, against any and all damages, claims and/or causes of action arising out of my participation and/or attendance at the Brazilian Martial Arts Center. 

I am not aware of any medical, physical and/or mental condition that would preclude my participation in martial arts training and/or competition.
 

I waive any monetary or similar compensation for and authorize the use of my likeness, including but not limited to written statements, video, audio and/or photographic media obtained of and/or from me while I am a participant in any training and/or competition conducted by, at or on behalf of the Brazilian Martial Arts Center.
 

I acknowledge that martial arts participation, training, attendance and/or competition conducted at Brazilian Martial Arts Center involves the adherence to the sports customary and usual rules and regulations. I agree to abide by said rules and regulations and acknowledge that my failure to abide by said rules and regulations will result in my immediate dismissal from the Brazilian Martial Arts Center without any monetary recourse to me from Marcelo Siqueira, the Brazilian Martial Arts Center, its agents, servants, and/or employee. 
 

I authorize the Brazilian Martial Arts Center to send me marketing messages via email and text message. 
 

I have read and understand the foregoing. I acknowledge that this document is a binding legal contract and, as such, I have been advised by the Brazilian Martial Arts Center to consult with an attorney prior to agreeing to this agreement.
